Checklist of Basic Documentary Requirements
• Approved membership status verification slip (valid for 6months)
• Two (2) pcs 1×1 ID picture
• Residence Certificate (cedula) – valid on the same calendar year
• Photocopy of 2 valid IDs
• Additional documents based on buyer’s civil status
• Proof of Billing
• Birth Certificate (if co borrowers included in computation of aggregate income)
• Post dated cheques (12PDC’s for DP ; 12 PDC’s for MA)
Proof of Income Documents
Locally Employed
• Latest ITR with attached W2 form stamped received by BIR or BIR form 2316
• Notarized Certificate of employment and compensation employed format plus 1month pay slip (if government employee)
• Other income documents that the PAGIBIG may prescribe
Self Employed
• ITR/audited Financial Statement/OR of Tax payment to bank
• DTI registration /Mayors permit
OFW Members
• Employment contract (EC) with English translation if written in foreign language
• Certification of income duly certified by employer
